The Certificate of Candidacy will be used as a reference whether a candidate is permitted to move on the screening process.
Only one (1) Certificate of Candidacy is allowed per candidate.
Candidates shall use the platform sejda.com in editing their Certificate of Candidacy.
A candidate is officially entered the waiting list for the screening process once they submitted the PDF file of their Certificate of Candidacy through the given Google Form.
Multiple submissions of a candidate’s Certificate of Candidacy will not be accepted and will result into disqualification.
Distorted Certificate of Candidacy and files that are not in PDF form will not be accepted and will be asked to submit a new one.
Any tampered/incorrect information found in the Certificate of Candidacy of a candidate shall lead to them being disqualified.
The deadline of submission will be on June 6, 2022 at exactly 8:00 AM.
Any late submission of Certificate of Candidacy, regardless of any reason, will not be accepted.

Click the "Certificate of Candidacy" button to download the PDF file that you will use for your Certificate of Candidacy.
Once downloaded, click the "sejda.com" button and you will be directed to website where you will edit your Certificate of Candidacy.
In the website, click the "Edit a PDF Document" button.
Click "Upload PDF file", find and select the downloaded copy of Certificate of Candidacy, and press "Open".
Select "Text" and double click to the boxes/lines where you will input your answers.
To add your signature, press "Sign", then "New Signature", and "Upload Image".
Note: Make sure to have a digital copy of your signature in your device.
Select the original version of your signature if it doesn't have any background. If it has a background, select the transparent version.
Left-click it to add into the pdf file.
Once done, press the "Apply changes" button and wait for your Certificate of Candidacy to be ready.
Press the "Download" button to get your Certificate of Candidacy and submit it through the Google form below.
